The Derbyshire Home Not Hospital Service Provides Low Level Support To Prevent Hospital Admission Or To Support With Hospital Discharges. The Service Supports People Who May Not Be Eligible For Social Care / Nhs Support But Whose Needs Are Likely To Escalate Without Any Intervention. The Service Offers Initial Practical Support For Up To 6 Weeks To Help The Individual As They Are Recovering To Remain Living At Home Independently.
