⑴ Official in charge of disbursement of the procuring entity : Hideaki Isohata, Director of Accounting Department, Self Defense Forces Central Hospital
⑵ Classification of the services to be procured : 78
⑶ Nature and quantity of the services to be purchased : Medical waste disposition. 120,000㎏
⑷ Details of this services : Refer to manual for tender and specifications.
⑸ Fulfillment period : From 1, April 2022 to 31, March 2023
⑹ Fulfillment place : Self Defense Forces Central Hospital
⑺ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall :
① Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Person under Conservatorship or Person under Assistance that consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause
② Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting
③ Have Grade A, Grade B or Grade C on "Offer of services" in the Kanto and Koushinetsu area in terms of the qualification for participating in tenders by Cabinet office. (Single qualification for every ministry and agency) in the fiscal years 2022, 2023 and 2024.
④ Not be currently under a suspension of business order as instructed by the Ministry of Defense.
⑤ It should not be parties concerned of the crime syndicate that receives the exclusion request from the administrative divisions police.
⑻ Time limit for tender : 17 : 15, 14 February 2022 in case of by mail : 10 : 00, 15 February 2022 in case of by hand.
