The Aberdeen City health and social care partnership has decided to recommission the delivery of care and support services at home for adults and has adopted a locality d, outcomes-focused contract to achieve the following aims:

— to meet individual and system wide outcomes, including the aims of the strategic plan;

— to ensure market stability;

— to deliver care and support efficiently and effectively, including through the utilisation of digitalisation and technology assisted care;

— to ensure delivery is aligned to the medium and longer term financial plan.
