Midlands Partnership NHS foundation Trust (MPFT) is looking to procure providers who are able to provide a care and support at home. These services forms part of a suite of integrated prevention and discharge to assess (D2A) ‘Home First’ services across Staffordshire and Stoke on Trent and include provision of Reablement service (up to 6 weeks), short term interim home care (pre/post reablement) and night sits. Services are lotted by requirement and location (providers are not require required to provide all services).
