2. You may download the Request For Quotation form or you may get it at BFAR BAC Secretariat Office, 3rd flr. PCA Main Bldg, Elliptical Road, Diliman, Quezon City.
3. PRICE QUOTATIONS THROUGH EMAIL WILL BE ACCEPTED AS YOUR FORMAL BID QUOTATION UNTIL Community Quarantine IS LIFTED.
4. Please fill up/use the Request for Quotation found in the “Associated Components”, otherwise your bid will not be accepted/considered.
5. All entries in the RFQ must be TYPEWRITTEN and LEGIBLE. (AVOID erasures and use of correction pens)
6. Suppliers MUST submit the ff;
• PhilGEPS Registration Number
• Mayor's/Business Permit
• Income/Business Tax Return
• Omnibus Sworn Statement
7. Place the RFQ in a SEALED envelope and type the following details on the face of the envelope.
• Company Name
• RFQ No.
• PR No.
• PhilGEPS Reference No.
8. Please fill up all the necessary information in the Supplier/Contractor/Consultant’s Certification Box.
9. Quoted Prices must be inclusive of taxes and other charges or fees and shall NOT EXCEED THE APPROVED BUDGET FOR THE CONTRACT.
10. FAILURE TO COMPLY with the above-mentioned conditions shall mean DISQUALIFICATION of the bid proposal.
11. We will NOT ACCEPT accomplished RFQs which are submitted PAST the deadline.
