INSTRUCTIONS/TERMS AND CONDITIONS:
1. The Bidders/Suppliers may submit their own canvass form filled up with the offered brand, unit price and total price.
2. Quotation/s must include all kinds of taxes for the item(s)/service(s) listed hereunder, including delivery charges.
3. Always indicate the brand name/model of the offered item(s) or product.
4. Price quotation(s) submitted shall be valid for a period of THIRTY (30) days reckoned from the deadline for submission of
quotations as stated above.
5. Quotation(s)/Proposal(s) shall be addressed & submitted to the Bids and Awards Committee thru the BAC Secretariat.
6. Quotation(s)/Proposal(s) should not be tampered/altered. Only sealed canvasses shall be considered by the BAC.
7. Awarding shall be done by LOT (meals/snacks/accommodation) and by ITEM (other procurement).
8. DEADLINE FOR SUBMISSION OF BIDS - closing date and time as stated above.
9. Delivery Period/Effectivity Period: Seven (7) working days from receipt of approved Purchase Order (PO)/Work/Job Order (JO).
10. Availability of Stocks: 60 days reckoned from instruction no.8.
11. Validity of Price is 30 days from receipt of PO/JO.
12. Mandatory Requirements: (must be submitted together with the Request for Quotation)
Certified True Copies of Mayor's/Business Permit and PhilGEPS Registration Number
13. The bidders shall reflect on the envelope the Solicitation No. and the PRAS No. The BAC shall not be responsible
