3. Latest Income / Business Tax Return duly stamped and received by the BIR and with validated tax payment
4. Notarized Omnibus Sworn Statement (prescribed revised format attached)
- Duly Secretary's Certificate if issued by the Corporation, Board/Partnership Resolution if partnership and/or
joint ventures, or Special Power of Attorney (SPA) in case of sole proprietorship for situations where the signatory is not the sole proprietor/owner
