Please quote your lowest price for the following items enumerated below, taking into consideration the following:
TERMS AND CONDITIONS:
1. The Approved Budget for the Contract (ABC): P 468,300.00 .
2. Price Quotation should be inclusive of the 5% Final VAT and expanded withholding tax (EWT) of 1%.
3. Price Quotation shall be valid within fifteen (15) calendar days from the deadline of submission of RFQ.
4. Delivery period is 45 calendar days from the date of Award/Purchase Order.
5. If awarded the contract, processing and payment shall be made after complete delivery of services/supplies and final acceptance of services.
6. Refusal to accept an award may be a ground for imposition of administrative sanctions under Rule XXIII of the Revised IRR of RA 9184.
7. Quotations must be delivered at EMB-CAR, DENR Cpd., Gibraltar Rd., Baguio City or send thru fax at (074) 446-6440 c/o the BAC.
8. The procuring entity may terminate the contract upon prior notice, in whole or in part, at anytime for unsatisfactory service.
9. Bid/Quotation may be typewritten/handwritten and should be placed in a sealed envelope marked " IT and Office Equipment/Accessories. "
