The University of the Philippines, System (UPS) hereby informs the public that it will be procuring the following item/s through Small Value Procurement:

Email your quotation signed by you or your duly authorized representative, with the eligibility requirements listed below, to the Procurement Office email address, bsd@up.edu.ph not later than Wednesday, May 19, 2021@10:00 AM.
• Photocopies of the following documents duly certified by the authorized representative:
• 2020 Mayor’s/Business Permit or its Equivalent Document and Proof of Payment for 2021 Mayor’s/Business Permit; or 2021 Mayor’s/Business Permit
• Income Tax Return for 2020;
• PhilGEPS Registration Number;
• Original Copy of:
• Conformity with the Technical Specifications
• Financial Proposal Form (Attached as Annex A)
• Notarized Omnibus Sworn Statement (Attached as Annex B)
• Authority of Signatory (Attached as Annex B1 or B2 whichever is applicable)
Please comply with the attached Supplemental Guidelines for Online Bid Submission. Receipt of the Bid Submission shall be subject to verification on strict compliance with said Guidelines.
