In view of the above activity, we are inviting external providers to submit their lowest price(s) on the services needed. Please read the instructions below.
Training Methodology:
1. Webinar
2. Presentation of learning videos
3. Workshops
4. Discussions/Open Fora
Please see attached table for the following:
1. Deion of Training/Seminar/Consultancy Services
2. No. of Participants per Training Program
3. Approved Budget per Training Program
4. Schedules of Training/Seminar/Consultancy Services
INSTRUCTIONS:
1. Prospective bidders are required to read the instructions and fill out the blanks properly and required to use the official canvass form in accomplishing their proposals/quotations.
2. Any specification other than those required/stated in this form shall not be considered in the evaluation of bid.
3. Quotation/s must include all kinds of taxes for the item(s)/services listed in the RFQ, including delivery charges.
4. Price quotation(s) submitted shall be valid within NINETY (90) days from the deadline for submission of quotations.
5. Terms of payment within Thirty (30) days from delivery of services.
6. Quotation(s)/Proposals including Training/Seminar outline or module for each Training/Consultancy service shall be submitted to the Office of the BAC Chairperson, or you may submit your proposal to the electronic mail of BAC. (DOSTRegion2BAC@gmail.com)

7. For those who will submit hard copy, bidder is required to affix his/her signature across the flap of the sealed envelope.
8. Bids received in excess of the ABC shall be automatically rejected at bid opening.
