The Provincial Government of Tarlac (PGT) through its Bids and Awards Committee, intends to procure “SUPPLY OF FUEL FOR VARIOUS RP SERVICE VEHICLES OF SP MEMBERS” for CY 2021 in accordance with Section 53.9 (Negotiated Procurement – Small Value Procurement) of the 2016 Revised Implementing Rules and Regulations of Republic Act No. 9184.

Submit your quotation duly signed by your authorized representative not later than June 22, 2021, 10:00 a.m. at the Bids and Awards Committee Office, 3rd Floor, Provincial Capitol Building, Brgy. San Vicente, Tarlac City.

The following eligibility requirements are required to be submitted along with your signed quotation/proposal:

a. Original Brochures or certifications (if applicable) of the items offered showing its performance characteristics, including names and addresses of firms providing service facilities in the country (if any)

b. Valid Mayor’s/Business Permit

c. PhilGEPS Registration Number

d. Latest Income/Business Tax Return (for ABCs above P500k)

e. Omnibus Sworn Statement duly notarized (for ABCs above P50k)
