⑴ Contracting Entity : Keiji Sato The Obligating Officer, Director Administration Department, Japan Organization of Health and Safety Chubu Rosai Hospital

① Magnetic Resonance Imaging System, 1 Set
② Lease of endoscopic surgery system (VPP)
⑷ Fulfillment period :
① by 31, March, 2022
② From 1, October, 2021 through 31, September, 2026
⑸ Fulfillment place : Chubu Rosai Hospital
⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall :
① not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting, Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause,
② not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting,
③ have Grade A or B or C or D in ① "Sales of products" ② "Offer of services etc" for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Tokai-Hokuriku Region,
④ meet the qualification requirements which the Obligating Officer may specify,
⑺ Time-limit of tender :
① 16 : 00 20, August, 2021
② 16 : 00 25, August, 2021
