⑴ Official in charge of disbursement of the procuring entity : Toshimitsu Yamada, Director, Worker's Compensation Administration Division, Worker's Compensation Department, Labour Standards Bureau, Ministry of Health, Labour and Welfare
⑵ Classification of the products to be procured : 26
⑶ Nature and quantity of the products to be required : Purchase of mobile devices to be used with the Asbestos Notification/Reporting System.
⑷ Delivery period : 10 March 2022
⑸ Delivery Place : The place specified by the contracting entity.
⑹ Qualifications for participating in the tendering procedures, Supplier eligible for participating in the proposed tender are those who shall :
① not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Persons under Conservatorship or persons under Assistance who have obtained the consent necessary for concluding a contract May be applicable under cases of special reasons within the said clause.
② not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting.
③ have Grade "A", "B" or "C" in terms of the quallifications for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in the fiscal year of 2019, 2020 and 2021.
④ For bidders subject to the following insurance system of each item, and it is not in arrears either of insurance premiums (2 insurance year about "e" and "f") for the last two years of the bid submission deadline of this bid :
a Welfare annuity insurance
b Health insurance (the thing wicth association of national health insurance has jurisdiction over)
c Seamen's insurance
d National pension
e Worker's compensation insurance
f Unemployment insurance
⑤ prove to have no false statement in tendering documents.
⑥ prove neither the business condition nor credibility is considerably deteriorating.
⑺ Time-limit for tender : 4 : 00 PM, 27 September 2021.
