⑴ Official in charge of disbursement of the procuring entity : MIURA Eiichirou, Director of General Coordination Department, Aichi Labor Bureau
⑵ Classification of the products to be procured : 14, 71, 27
⑶ Nature and quantity of the products to be purchased : CAD system machine' lease
⑷ Lease period : From 1 March 2022 through 31 March 2026
⑸ Delivery place : Refer to tender instructions
⑹ Qualifications for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall :
① Not come under the provisions of Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, persons under curatorship or assistance that obtained the consent required for concluding a contract may be applicable under cases of special reasons within the said clause,
② Not covered by Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting,
③ Have Grade "A", "B" or "C" on "provision of services" in terms of qualification for participating in tenders by the Ministry of Health, Labor and Welfare (single qualification for every ministry and agency) in the fiscal year of 2019, 2020 and 2021,
④ Not be behind in payment of the premium for latest two years (the latest two insurance years as for (ⅴ) and (ⅳ)), such as :
(ⅰ) Employees' Pension Insurance
(ⅱ) Health Insurance (managed by the Japan Health Insurance Association)
(ⅲ) Seamen's Insurance
(ⅳ) National Pension
(ⅴ) Workers' Accident Compensation Insurance
(ⅵ) Employment Insurance
⑤ Not be under the period of business suspension order as instructed by the Ministry of Health, Labor and Welfare,
⑥ Prove neither the business condition nor credibility is considerably deteriorating,
⑦ Prove to have no false statement in the tendering documents to be submitted,
⑧ Not have received any administrative disposition within the past year for violating the laws and regulations of the Ministry of Health, Labor and Welfare,
⑨ Meets the qualification requirements stipulated by the Director of General Coordination Department in accordance with Article 73 of the Cabinet Order concerning the Budget, Auditing and Accounting.
⑺ Time-limit for submission of certificates : 5 : 00 PM, 20, October, 2021
⑻ Time-limit for tender : 3 : 00 PM, 21, October, 2021
