⑴ Official in charge of disbursement of the procuring entity : MAKI Shuichiro, Director, General Affairs Division, Beppu Severe Physical Disabilities Institution, Rehabilitation Services Bureau, National Rehabilitation Center for Persons with Disabilities
⑵ Classification of the products to be procured : 71, 27
⑶ Nature and quantity of the products to be purchased : Purchase and setting of equipment related to the premises LAN system 1 set
⑷ Fulfillment period : From a contract day to March, 31, 2022
⑸ Delivery place : Beppu Severe Physical Disabilities Institution, Rehabilitation Services Bureau, National Rehabilitation Center for Persons with Disabilities.
⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall :
① not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Persons under Conservatorship or persons under Assistance who have obtained the consent necessary for concluding a contract May be applicable under cases of special reasons within the said clause.
② not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting.
③ have Grade A or B or C, on "Sale of Products" or "Offer of Service" in the Kyushu-Okinawa Area in terms of the qualification for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in fiscal years 2019, 2020, 2021.
④ Are not being suspended from transactions by request of the officials in charge of contract
⑤ Prove to have no false statement in tendering documents.
⑥ Prove neither the business condition nor credibility is considerably deteriorating.
⑦ For bidders subject to the following insurance system of each item, and it is not in arrears either of insurance premiums (2 insurance year about Ⅴ and Ⅵ) for the last two years of the bid submission deadline of this bid :
Ⅰ Welfare annuity insurance
Ⅱ Health insurance (the thing which association of national health insurance has jurisdiction over)
Ⅲ Seamen's insurance
Ⅳ National pension
Ⅴ Worker's compensation insurance
Ⅵ Unemployment insurance
⑧ Meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabinet Order.
