⑴ Official in charge of disbursement of the Procuring entity YAMADA Toshimitsu Director, Worker's Compensation Administration Division Worker's Compensation Department, Labour Standards Bureau, Ministry of Health, Labour and Welfare
⑵ Classification of the services to be procured : 71, 27
⑶ Nature and quantity of the products to be purchased : Set of printing consignment of the notice of payment
⑷ An observance of a contract period : From 1 April 2022 to 31 March 2023
⑸ An observance of a contract place : The place specified by the contracting entity
⑹ Qualifications for participating in the tendering procedures, Supplier eligible for participating in the proposed tender are those who shall :
① Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, Persons under Conservatorship or Persons under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause.
② Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting.
③ Have Grade ｢A｣ or ｢B｣ on "Offer of service" in terms of qualification for participating in tenders by Ministry of Health, Labour and Welfar (Single qualification for every ministry and agency) in the fiscal years 2019, 2020 and 2021
④ For bidders subject to the following insurance system of each item, and it is not in arrears either of insurance premiums (2 insurance year about (ⅴ) and (ⅵ)) for the last two years of the bid submission deadline of this bid :
(ⅰ) Welfare annuity insurance
(ⅱ) Health insurance (the thing which association of national health insurance has jurisdiction over)
(ⅲ) Seamen's insurance
(ⅳ) National pension
(ⅴ) Worker's compensation insurance
(ⅵ) Unemployment insurance
⑤ Prove to have no false statement in tendering documents.
⑥ Prove neither the business condition nor credibility is deteriorating.
⑦ Meet the qualification requirements which the Obrigating Officer may specify in accordance with Article 73 of the Cabinet Order
⑺ Time-limit for tender : 4 PM 21 December 2021
